I will show you how to create a basic Contact Form with Caldera Forms – a free yet extremely powerful form creation plugin that provides complete interface and functionality.
1: Installation: you go to Plugins => select Add new and then type the keyword “Caldera Forms”:

You go to Caldera Forms => New form. Here are a few templates you can choose from
Here I choose Blank page and name it:
Now it’s the Add field section, it has many options for you:
Select the field and then edit the right part as shown:
Now comes the “Form Settings” section:
+ Form Name: form name
+ Scroll to top on submit: when clicking submit button will scroll to the top or not.
+ Success Message: Notify when successfully submitted, here there are additional value fields such as %name% (it is the name of the field name that we set the fee above).
Go to the Email section:
From Email: email from the sender.
Reply to Email: email from the recipient.
Once done, you can copy the shortcode and paste it into a page or post to see the results.
